Coronavirus Advice

a) Please SMS to confirm your face-to-face appointment.

b) Please call us if you want a Telehealth (phone) appointment

**Please read instructions below before your appointment.**

a) For Face-to-face Appointment:

If you are attending the appointment, please do the following:

  1. Come to appointment alone where possible.
  2. Carpark code 25802.
  3. Wait in car until appointment time ready, phone 49632323, option 1 to notify you have arrived.
  4. Reception will call you when ready to come inside for appointment. Only the patient is to come into the Centre.
  5. Temperature check will be applied on arrival and hand sanitizer will also need to be applied.

Please note that, as per NSW Gov mandate, our staff will be wearing face masks during your appointment.

b) For Telehealth Appointment:

We strongly encourage high-risk patients to opt for a telehealth appointment. High-risk patients include:

  • at least 70 years old; or
  • at least 50 years old and is of Aboriginal or Torres Strait Islander descent; or
  • is pregnant; or
  • is a parent of a child under 12 months; or
  • is already under treatment for chronic health conditions or is immune compromised

For patients with diabetes who wear an insulin pump, please download to Carelink prior to your appointment. For appointments with our Diabetes Educator Annette or Dietitian Nicole – you will still need a visit on your care plan, which you can get from your GP over the phone

 

IMPORTANT: If you have ANY of these symptoms:

  • fever
  • cough
  • sore throat
  • shortness of breath AND
  • travelled internationally in the 14 days before getting sick OR
  • had contact with someone diagnosed with or suspected of having coronavirus in the 14 days before getting sick.

Please do not enter the practice.

This will help protect the health and safety of you and others. A phone appointment will be arranged instead if necessary.
